A referral should create less work for you, not more.

When you're sourcing direct support, you need enough information to know whether the provider is genuinely suitable. VIVO aims to make that straightforward: clear scope, honest capacity, direct communication and no pretending a referral is a fit when it isn't.

We understand that your role includes helping participants connect with providers that suit their goals, needs and plan. Our job is to give you a clear picture of what VIVO can actually provide.

01Direct communication

Speak with the person responsible for intake and current service delivery rather than being passed through layers of administration.

02Honest capacity

If the requested hours, needs or location aren't a suitable fit, we'll say so rather than accepting a referral we can't support properly.

03Information actually used

Relevant referral information, reports and participant-specific guidance are reviewed to understand the person and the requested support.

04Communication after commencement

With appropriate consent, relevant changes, concerns and useful observations can be communicated to the wider support team.

Direct support often sits between appointments, therapy sessions and planning conversations. With participant consent and where appropriate, VIVO can help keep everyday support informed by relevant professional recommendations and participant-specific strategies within our role.

Important: VIVO is a direct disability-support provider. We do not present ourselves as the participant's clinician, Support Coordinator or Specialist Behaviour Support Practitioner.

The Referral Process

What happens after you refer.

A referral starts a conversation, it isn't an automatic acceptance. We first establish whether VIVO is the right provider for the person and the requested support.

Send the enquiry through VIVO's referral/contact pathway or contact Cade directly.

Initial review

We review the support requested, location, hours, funding arrangement and relevant context.

Conversation

We speak with the participant, representative or referrer to clarify needs and what good support should look like.

Fit confirmed

Capability, capacity, relevant information and service arrangements are considered before commencement.

Support begins

If the fit is right, introductions and service arrangements are completed and support can commence.

Support Coordinators and other professionals need enough information to help participants compare providers. VIVO won't ask a referrer to present us as the only option or imply that a participant has to use VIVO.

No referral incentives. No pressure.

VIVO's role is to explain our services, availability and fit clearly. The participant remains free to choose the provider that best suits them. If VIVO isn't the right fit, we'll say so.

Which participants can I refer to VIVO?
VIVO considers referrals for plan-managed and self-managed participants seeking one-to-one disability support across Bundaberg and the wider Wide Bay region. We have particular experience around psychosocial disability, mental health and more complex support needs, while also supporting people with autism, neurodivergence, intellectual disability and other individual support needs where the referral fits our capability.
Can I speak with VIVO before submitting a referral?
Yes. Professionals are welcome to call or email first to discuss current capacity and whether the requested support appears to fit VIVO's service scope before sending detailed participant information.
Will VIVO work with the participant's existing professional team?
Yes, where appropriate and with relevant consent. VIVO can communicate with Support Coordinators, Psychosocial Recovery Coaches, Behaviour Support Practitioners and allied health professionals involved in the participant's supports.
Can VIVO support a participant with a Behaviour Support Plan?
Potentially. This depends on the participant's needs, the contents of the plan, the support requested, VIVO's capability and applicable requirements. VIVO does not provide Specialist Behaviour Support, conduct Functional Behaviour Assessments or develop Behaviour Support Plans, and does not use regulated restrictive practices as an unregistered provider.
Does VIVO guarantee acceptance or a specific commencement timeframe?
No. We prefer to be accurate rather than advertise a blanket turnaround promise. Availability and commencement depend on the participant's needs, location, requested hours, required preparation and VIVO's current capacity.
Who will I communicate with?
VIVO is currently founder-led. Cade is directly involved in intake, professional communication and current support delivery. As the participant base grows and additional workers are introduced, VIVO's aim is to keep communication clear and responsibility identifiable.
